Care Referral Form

If you believe that this is an emergency situation, including an imminent threat of harm to self or others, please call the UNC Police Department at 919-962-8100 or call 911. The submission of a Care Referral Form will not address an emergency situation. 

You are encouraged to submit a Care Referral Form to report any concerns you have for a student. You referral will be assigned to a member of staff in the Dean of Students office for review and response within two business days. Please include as much relevant information as possible. 

If your concern involves discrimination and harassment, including sexual and gender-based harassment, sexual violence, interpersonal violence, or stalking, please submit an EOC Report Form.

If you would like to speak with a staff member during normal business hours to discuss your concern, please call 919-966-4042.

Final Exam Excuses

The Dean of Students office, including the University Approved Absence Office (UAAO), does NOT issue University Approved Absences or Final Exam Excuses for final exams or dates occurring during the final exam period. 

DO NOT SUBMIT AN ONLINE REQUEST FOR UNIVERSITY APPROVED ABSENCES FOR FINAL EXAMS.  

If you will miss a final exam due to a medical or mental health concern, family or personal emergency, religious observance, or exam schedule conflict, please follow the instructions on page linked below and communicate directly with your instructor to schedule a make-up.
